SUMMARY Management Business Solutions is seeking a Social Worker for its client in Riverside CA. The person will be responsible for providing support to individuals by conducting assessments developing care plans and collaborating with a team to ensure their needs are met effectively. RESPONSIBILITIES Conduct comprehensive assessments to understand the needs and preferences of individuals. Collaborate with a multidisciplinary team to create personalized care plans. Facilitate access to necessary services and resources for individuals. Ensure smooth transitions between different care settings. Lead and organize care planning meetings to coordinate support efforts. Develop discharge plans for individuals leaving the program. QUALIFICATIONS Master s Degree in Social Work (MSW) is required. 1 years of experience working with elderly or frail populations. Proficiency in organizing and leading meetings.
