Companion Coordinators-Live In

The Live In Companion Coordinators provide in-home direct care services for adults with intellectual disabilities in a home setting as well as day to day supervision including direction and mentoring of assigned staff and is responsible for addressing the urgent issues that evolve regarding the service recipients and the homes. This is an opening for a couple who will live in a home with 3 service recipients. This position implements monitors and reports on directives of the program supervisor. The Companion Coordinators will be off every third weekend (Friday from 9 00 am to Monday at 3 00 pm) and would also have free time Monday through Thursday from 9am-3pm. Rent utilities and basic food staples are paid for by Prospect. Only one coordinator works per shift therefore there is a day shift and a night shift. The Coordinator that works night shift must remain awake alert and able to care for the service recipients throughout the night. Duties and Responsibilities include but are not limited to Maintain a safe and positive work environment. Day to day supervisor for the assigned houses. Mentor Direct Support Professional staff. Provide formal on the job training to DSP (caregivers) staff in the home. Complete cross training of new employees and required training documentation Individual Specific Training Fire Drill Training and two hour job shadows.) Provide employee performance feedback and conduct performance reviews. Conduct job applicant interviews and job previews. Address and resolve urgent issues that are related to the service recipients and the home. Responsible for carrying cell phone and being the first one to respond to staff when issues arise. Represent Prospect and the Residential Supervisor at circle of support meetings ISP meetings and all meetings that are regarding the home and the service recipients. Take service recipients to medical appointments. Responsible for ensuring staff are clear on all directives from the supervisor and all information regarding the service recipients and the home. Contact the Accounting Assistant to ensure funds requested by service recipients are available and pick up their checks from the Accounting Assistant. Shop for groceries for the homes with the service to teach shopping skills and ensure expenditures are within shopping budget. Coordinate service recipient errands and complete as needed. Responsible for medication administration record coordination. Maintain medication levels in the homes to ensure that no service recipient has less than a five-day supply of refillable medications. Review Daily Notes and service recipient Therap Documentation. Prepare and complete the Service Recipient Review of Services (used to be called the monthly review currently the review is completed every other month.) Review and approve menu plans. Review pre-trip vehicle inspections to ensure any identified issues are promptly addressed. Involve service recipients in different activities community activities and chores as appropriate. Teach and record goals and outcomes that are to be implemented at home and in the community. Ensure service recipients are well groomed and appropriately dressed each day. Ensure the house is well maintained and clean. Each shift has cleaning responsibilities that must be completed. Follow Prospect s transportation policies and procedures and vehicle operating procedures. Ensure service recipients are not left unsupervised in cars vans home and or community unless it is specified in their Individual Support Plan (ISP). Ensure monthly paperwork is completed and returned to the RSS by the 3rd of each month. Position Minimum Requirements High school diploma or equivalent Previous experience in a leadership role Previous related experience and or equivalent combination of experience and relevant post high school education Completion of Prospect Inc s Leadership 101 course and CPI training. Ability to communicate clearly and effectively in English with service recipients and staff both in writing and verbally Ability to use good judgment Ability to follow specific written and verbal instructions Ability to do basic math Ability to work any shift for assigned days and overtime Valid Tennessee driver s license with good driving record Must be able to pass all background tests and drug screens.
