Office orientation hours Patient orientation hours and Overtime Holiday hours do not count to the bonus payout. 50 % of Bonus will be paid out on the next pay cycle after the first 30 regular hours are completed. Remainder of Bonus will be paid out on the next pay cycle after the next 30 regular hours are completed. If employee is separated from employment due to termination misconduct or non-completion of a 2 week notice after first 50% of bonus is paid the partial paid signing bonus amount of 250 will be deducted from any remaining paycheck of employee. Once a sign on bonus is completed if employee separates from IntelliChoice Home Care they will not be eligible for any recruitment candidate attraction bonuses for the period of 365 calendar days from date of separation. Employee will forfeit signing bonus under the following conditions Employee no calls no shows to a scheduled assignment. Employee calls out for a scheduled shift less than 4 hours prior to accepted shift. Employee calls out for a shift with an unexcused absence. Only excused absences will be death of family member medical emergency (self of immediate family) or extreme circumstances approved as IntelliChoice Management (natural disaster etc).
