Location Elmhurst NY Description We are seeking compassionate Certified Home Health Aide (HHA) to work within our clients in cluster care environment providing support with daily living tasks under the supervision of a Registered Nurse. Our qualified Certified Home Health Aide (HHA) work closely with patients and are responsible for basic care services such as bathing grooming feeding accompany to medical appointments etc. We are currently looking for Per Diem employees. Week Day Shifts Available (Mon-Fri) Days - 7am - 3 30pm Evenings - 3 30pm - 12 00am Nights - 11 00pm - 7 30am Weekend Shifts Available (Sat Sun) Days - 7am - 3 30pm Evenings - 3 30pm - 12 00am Nights - 11 00pm - 7 30am Qualifications - Current unencumbered state certification to practice as a Certified Home Health Aide (CHHA) - Minimum one (1) year of CHHA experience required based on state requirements. - Two (2) Professional reference letters - Current Physical (within 3 Months) and Drug test (within 30 days of hire) based on state requirements - Copy of Measles Mumps and Rubella Vaccinations - Copy of updated TB results Chest x-ray within 2 years or 2 Steps PPD within one (1) year - Permit to work in the US - Reliable transportation About us Queens Adult Care Center ALP is a modern and fully furnished 361-bed facility. It offers its residents comfortable accommodations delicious food and an array of creative and innovative programs. Located in Elmhurst in the New York City Borough of Queens this facility is near two urban shopping malls the recently expanded Queens Center and the Queens Place Mall. It s accessible to several nearby subway stations on Woodhaven Boulevard Grand Avenue Newtown and Elmhurst Avenue as well as the local Q53 Q58 Q59 Q29 and Q60 bus lines. Queens Adult Care Center ALP offers the New York State Assisted Living Program (ALP) This program is certified by the New York State Department of Health to provide Medicaid-funded home care services. The ALP is an alternative to nursing home that enables nursing home eligible residents to receive home health care services in the Adult Home delaying or preventing the need for admission to a nursing home. Care services are provided by an on-site licensed home care agency. At Queens Adult Care Center ALP we work hard to ensure an independent living environment where everyone can thrive and succeed.
