Hypertension Referral Program Manager - Part Time

Compensation 15- 25 per hour based on experience & education This role is funded through September 30th 2015 with the possibility of extension thereafter. Background The Boston Racial Ethic Approach to Community Health (REACH) Obesity and Hypertension Demonstration Project is one of two demonstration projects funded by the US Centers for Disease Prevention and Control (CDC) to implement and evaluate population-based policy systems and environmental changes that can support healthy behaviors and reduce the prevalence of obesity and hypertension among Black and Latino residents. Boston has been funded for three years at 4.6 million to develop implement evaluate and disseminate a portfolio of evidence-based strategies that build on past and current efforts for obesity and chronic disease prevention in the city of Boston. The Project is a collaborative partnership with the Boston REACH Coalition YMCA of Greater Boston and Harvard School of Public Health. REACH Hypertension Physical Activity Referral Program This program was developed as part of the REACH Hypertension strategy and consists of a referral pathway for patients with hypertension at Boston Community Health Centers to be eligible for free YMCA memberships. General Responsibilities Under direct supervision of the Director of Healthy Living and general oversight of the REACH Project Coordinator in the department of Healthy Living. Responsible for day to day duties of the Boston YMCA REACH Obesity and Hypertension Project to include Hypertension Physical Activity Referral Program Membership Management preliminary duties - Contact all Membership Directors Sales Teams with information regarding referral program. - Create a hypertension referral program flyer for branch distribution - Coordinate with membership support to set up appropriate membership types with cost and dates for programFacility Access Management - Run bimonthly reports (1st and 15th of month) to gather Facility Access data for all referral program memberships - Reach out to membership directors at each YMCA branch or to participant directly when participant has been identified as underutilized the facilities - Collect and enter referral membership data into excel spreadsheet to be shared with Boston Public Health Commisson Community Health CenterCommunication - Maintain communication with program participants - Collect information from membership directors as needed - Communicate with health associates on a regular basis - Communicate with Boston Public Health Commission and Community Health Centers as needed on a regular basis REACH responsibilities - Fulfill any additional REACH grant responsibilities as assigned - Attend Hypertension Strategy Committee meetings - Attend Hypertension Referral Program meetings - Attend internal YMCA REACH meetings Position Requirements Minimum Bachelors degree in public health or related field preferred. Experience in program management including budget grant management contract and supervisory responsibilities in public health community health or related field. Experience with health assessment program planning and development and program evaluation. Strongly prefer experience in managing initiatives that address policy and systems change. Familiarity with models of health outcomes based on social determinants of health and role of policy environmental and systems changes in health behavior change. Commitment to role of public health in promoting social justice and health equity. Experience and excellent skills in working effectively with diverse community populations Excellent organizational skills including ability to prioritize and to multi-task. Ability to work in a fast-paced team environment to meet deadlines and to flexibly adapt to multiple priorities. Ability to work effectively in a team approach to program management. Familiarity with Boston neighborhoods and Boston health and social service system preferred. Excellent verbal communication skills with experience in facilitating large meetings and public speaking. Excellent writing skills with extensive experience writing reports grants issue papers and related documents. Excellent analytic skills including ability to understand and interpret quantitative and qualitative data. Intermediate or higher level skill in Microsoft Office suite of Word Excel Powerpoint and Internet Explorer. Spanish language capability preferred.
