Home Health care experience is required with 1 year of hospital experience. Communicates effectively and timely with physician and notifies the care team of changes to the patient s plan of care. Responsible for planning coordination and management of patient care plan for assigned caseload. Completes and maintains current physician orders clinical communication and medication profiles for assigned patients. Flexible hours available and local travel is required. Current active RN License in the State of California by the Board of Registered Nursing. Current CPR health clearance and clinical skills self-assessment. Minimum of one year of professional nursing experience within the last 3 years.
