LPNs NEEDED for our Part-Time CARE MANAGER POSITION in the states of GEORGIA NORTH CAROLINA & SOUTH CAROLINA We are an Agency who Contract with Humana Senior Bridge Program. We do Care Management for their patients in a Home Health Setting. We are seeking nurses in the states listed above who has at least one year experience as a LPN. We allow each of our nurses to have 30 patients on their caseload. We ask individuals who are interested in working for us as a CM to have the ability to travel up to a 50 mile radius. We strive to assign clients who are in the areas you agree to service. You will need access to transportation a computer and a telephone to perform these services. Humana will do all of the training for this position. Training is done via internet. You will need to complete 4 modules and participate in a webinar. You will receive CEUs for completing the entire training. The pay scales we use for our CMs are as following Brief Assessments 50 per visit (this is a One Time Visit) Transitions 30 per visit (this visit is done 3 times per month) Long Term 30 per visit (1 visit per week) If this position is of interest to you please contact us at om2(at)qualityfirstinhomecare.com.
