Temporary - Refugee Health Case Manager

Temporary Employee (finishing on September 30 2015). Primary Purpose The Temporary Refugee Health Case Manager will partner with community healthcare providers to supplement existing services that do not adequately address the mental and physical health needs of refugees in Lancaster County and will provide case management services for arriving refugee cases with intensive health and or mental health issues. Essential Duties as Refugee Health Case Manager Provide intensive case management to refugees identified with special health needs to promote individual healthcare management in coordination with Resettlement Program case managers. Advocate and network within the mental and physical health community to educate the health community on refugee needs. Attend and participate in regional health conferences as outreach to the health mental health community. Coordinate with Resettlement Program case managers to identify vulnerable clients connect clients to support and make referrals to external service providers as necessary. Work closely with the Resettlement Program Coordinator to monitor program progress. Prepare and submit program assessment reports to document accomplishments and challenges. Coordinate with ethnic community members for interpretation services as needs arise in case management. Assist the Resettlement Program Coordinator and Lancaster Director in preparing reports as requested. Common Duties Advocate on behalf of refugees and refugee-serving voluntary agencies. Keep informed about refugee and immigrant issues and available services to refugees using current information to better identify and serve refugee clients. Represent the program at meetings and speaking engagements relevant to the responsibilities of the position. Undertake other duties as assigned. Qualifications Education Working toward bachelor s Degree in Healthcare Social Work or related field Experience Previous work experience and interest in refugee services is preferred. Ability to provide culturally-sensitive counseling and case management is required. Other Skills Superior written and verbal communication skills are required. The successful candidate must be computer literate with proficiency in MS Office (Excel Word and Access) and internet applications. Strong organizational and time management skills are required. Must be 21 years or older have a car and a have valid driver s license with a clean driving history.
