SPECIFIC RESPONSIBILITIES Clinical Assessments Conducts assessments of new patients using the identified format for documentation and clinical decisions regarding treatment interventions and referrals as instructed Assesses changing needs of patients per guidelines for outpatient and or IOP programs and incorporates these into treatment plan and treatment plan review Clinical Treatment With supervision and oversight is able to integrate into the overall counseling functions at the program and implements improves and adapts concepts models and constructs of treatment for application in service delivery to patients Creates a treatment plan within regulatory guidelines that includes an effective continuum of therapeutic supportive and other services for the patients serviced by the program Participates in regular treatment team meetings to discuss clinical cases review and update patient treatment plans monitor therapeutic progress and initiate recommendations for referral and continuing care Accepts assignments of patients with a variety and complexity of needs Provides individual and group therapy to patients in the program as instructed Engages families in the assessment and treatment processes when indicated Provides education to patients on recovery Supervision and Training Reviews serious clinical problems and cases with team members and notifies team of significant changes in clinical condition of patients Completes annual training needs assessment per program guidelines Referrals and Coordination of Clinical Care Discusses need for additional services with the clinical team to determine appropriateness of recommendations Refers patients to other levels of care when clinically indicated Refers patients to community based self-help programs for recovery as instructed Refers patients to other community social or human services as needed and advocates on behalf of patients to receive necessary services as instructed Documentation Documents all assessments treatment sessions contacts case management and other work done on behalf of patients according to guidelines Completes clinical documentation on the same day of service delivery Maintains daily records of clinical productivity in the program as instructed Provides required information about the patients according to confidentiality guidelines Provides clinical information to patient access representative personnel and insurance providers to ensure authorization for services Other Participates in the quality assurance and performance improvement activities of the program and agency including assisting in mentoring new hires Performs additional duties as assigned Performs in accordance with system-wide competencies behaviors Maintains professional licensure status as required by job description qualifications The above statement reflects the general details considered necessary to describe the principal functions of the job identified and shall not be construed as a detailed description of all of the work requirements that may be inherent in the job nor shall it be interpreted as limiting an employee in performing duties other than those normally performed or from helping employees in other position or units REQUIRED MINIMUM QUALIFICATIONS Masters degree in a generally recognized clinical discipline including but not limited to psychiatry social work psychology nursing or rehabilitation or activity therapies AND professional licensure status (LPC LCSW LMFT) AND five years of clinical experience.
