Myriddian is seeking an experienced Registered Nurse (RN) to provide coordinate and direct the provision of quality nursing care where patients most want to be in their homes. As a professional home health nurse with Myriddian you will join an elite team providing compassionate care excellent clinical outcomes and the area s highest patient satisfaction ratings. The territory covered by this position is Baltimore County. Responsibilities include Assesses patient conditions initiates plan of care with patient s physician re-evaluates and updates as necessary. Participates in obtaining all necessary physicians orders and collaborates with physician as patient s needs dictate. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical health errors. Takes part in case conferences to discuss multidisciplinary approach and reports patient progress plans for continued care etc. Plans for discharge from or recertification with agency. Makes recommendations for alternative level of care if necessary. Promotes patient family caregiver education using various verbal and written communication techniques that take into account the patient family s cultural ethnic and or personal needs or preferences. Supervises LPNs and HHAs as indicated in the Plan of Care. Initiates needed referrals to other disciplines and or community services. Obtains needed supplies and equipment to provide care. Participates in agency s mandated activities i.e. inservices commitee and staff meetings.
