SUMMARY Management Business Solutions is seeking Physical Therapists for its rehabilitation and nursing services client In Grand Rapids MI. The person will be responsible for overseeing home care services emphasizing expertise in physical therapy and community health education. RESPONSIBILITIES Administers physical therapy services assessing and evaluating patients therapeutic status and home environments. Initiates and directs physical therapy programs instructing personnel and families on goals through case conferences. Trains patients in prosthetic device use. Identifies additional home health service needs making referrals as required. Creates clinical and progress summaries based on goal attainment. Participates in discharge planning provides consultations and supervises assistants and home health aide care. QUALIFICATIONS Bachelor s degree (B.S.) from a four-year college or university required in Physical Therapy. A post-graduate degree is highly desired. State licensure as a Physical Therapist in Michigan required. Minimum of one (1) year of relevant experience as a physical therapist with a preference for experience in community home health settings required. Excellent verbal and written communication skills. Maintains up-to-date CPR certification is required. A valid driver s license with a insured automobile that complies with state and or organization requirements.
