Direct Support Professional

Kaleidoscope is seeking a dedicated Community Supports Worker to work with adults with disabilities. Community Based Supports Services are direct services provided to Participants who live with their families in their own homes or in the community with non-residential support services.Job Responsibilities Assisting individuals in achieving maintaining outcomes of increased independence productivity enhanced family functioning activities of daily living home maker and medical supports not otherwise covered by the Participants health plan and inclusion in the community as outlined in the ISP. Engage in activities with the individuals to allow them to live in the community to live more independently or to be more productive and participatory in community life. Adhere to the Supervisory requirement and attend weekly and monthly supervision. Job Requirements Be at least eighteen (18) years of age Must not have had any disqualifying crimes in a current BCI and or NCIC criminal background check and a Child Abuse check. Possess a high school diploma or general educational development (GED) certificate Current State motor vehicle registration is required for all vehicles owned leased and or hired and used as a component of service. Have and maintain a valid driver s license at all times Have a working knowledge of state and community resources available to Participants and their families and a knowledge of and ability to access these resources. Keep credentials updated with office personnel including Driving License Insurance Registration Criminal Clearances and other credentialing necessary to complete the requirements of the position. Complete and maintain CPR FA certification Medication Administration Certification and Crisis Prevention Intervention Training and techniques. Complete initial Pre-Service and 12 hours In-Service training annually as described in BHDDH policy.
