Family Services Assistant

The Family Services Assistant provides supportive services to all areas of the agency. This includes assistance with guardianship respite services funeral burial pre-planning human rights religious services and other needs as they arise.Responsible for assisting the Respite program to secure funding and to manage our licensed capacity. Chairs and manages the Human Rights Committee including advocating for the dignity and rights of the people served by Hattie Larlham. Created and completes trainings for members as required by DODD. Completes and presents annual analysis data as required by DODD.Completes Restrictive Measures Forms annually as required by DODD for any resident who requires restrictions including visitation medications mechanical etc. Assists families guardians with guardianship duties. Including helping the families complete and submit the annual guardian s report expert evaluation and annual plan. Assists families guardians in obtaining required ongoing education in accordance with state rule and Probate court requirementsManages database for client information. This includes demographic information updated medical diagnoses and functioning information required to be kept on current. Completes and submits IAF s electronically on a quarterly basis. Assists families in obtaining and maintaining appropriate funding for residential services as well as assisting families in maintaining up-to-date consent forms.If holding a degree in Social Work will participate as a member of the Interdisciplinary Team for caseload of residents. Coordinates and oversees family services in regards to end of life planning and or needs. Including obtaining managing irrevocable burial trusts organizing religious memorial services etc. Participates in Quality Improvement Committee projects and other projects as assigned.QUALIFICATIONS Bachelor s degree in human services field preferably in Social work from an accredited college or university. An Associate degree with experience may be considered.EXPERIENCE Experience in the area of developmental disabilities preferred. Must be knowledgeable in or willing to learn the ICF IID Medicaid Social Security and Licensure rules that are relevant to the position. Must be knowledgeable in various computer programs. Must be able to communicate well with families and community agency representatives.OTHER REQUIREMENTS Must be able to write and type reports and documentation. Must have valid Ohio driver s license and be able to travel to all Hattie Larlham service areas. Must be able to work some evenings weekends and holidays as needed. Must be flexible to meet the needs of both staff and the people receiving services.
