We are a very busy home health care agency servicing the Milwaukee area for over 30 years. The ideal candidate must be available willing to work days evenings and some weekends in all areas of Milwaukee. You may work as much or as little as you want full or part time.Responsibilities include Assessing new clients for admittance to the AgencyCompleting initial Oasis forms and re-certifications as needed.Maintaining client medication records and noting changes accordinglyCommunication with the client s PCP regarding changes in conditionsEducating clients regarding their disease etc.Professionalism is a must. You must have your own transportation and be willing to work in all areas of Milwaukee. RN CM s are compensated per visit this part time position could easily become full time depending on how much you would like to work.Our office is very relaxed and strives to make paperwork as easy as possible to enable our nurse s to remain in the field. We have a staff of LPN s available to help out with visits as needed. The option of case sharing is also available however we are specifically looking for an RN willing to open and follow new clients.The ideal candidate may already be working with another Agency and may be looking for more work.
