AltaMed Health Servcies - El Monte CA Performs professional work in the application of Medical Nutrition Therapy (MNT) to meet the dietary needs of AltaMed patients. Dietitian will work with high-risk patients as identified by the specific program or department Requirements Registered Dietitian. Registered by the American Dietetic Association required. A minimum of one (1) year professional work experience in clinical nutrition as Registered Dietitian. Bilingual English Spanish Required due to population being served CPR and First Aid certification required Responsibilities Determines dietary needs of patients from charts and other sources. Calculates special diets ordered by the physician. Prescribes therapeutic diets for high-risk patients and performs reassessments as necessary. Provides patient specific detailed education to patient family caregiver regarding nutritional status Develops daily or weekly menus for participants that meet nutritional requirements. In conjunction with the program director conducts classes in nutrition as related to diseases i.e. weight control classes diabetic classes etc. Monitors adherence and response to nutrition therapy adjusts therapy as needed to achieve and sustain effective nutritional status.Maintains and keeps record on consultations and assessments given to participants via EHR. Provides all dietitian services on the premises at appropriate times on a regularly scheduled basis. A written record of the frequency nature and duration of the visits shall be maintained. Provides general technical assistance and consultation to various AltaMed programs or departments (e.g. Health Education Women s Health Obesity Prevention and HIV services). Assists in assessing planning implementing and evaluation nutritional program(s) in the clinic. Performs other related duties as assigned.
