HOME CARE PT WANTED Summary The Physical Therapist is a registered professional who provides physical therapy to patients of all age groups. The Physical Therapist plans implements and evaluates patient care plans to restore or maintain patient well being. The Physical Therapist plans organizes and conducts physical therapy treatment program based on the medical referral and their evaluation. The Physical Therapist collaborates with interdisciplinary team. Essential Functions The Physical Therapist performs clinical assessment to identify patient needs and appropriateness of patient. The Physical Therapist documents prepares reviews and completes a Plan of Care (POC) progress notes phone orders and other clinical record documentation on an ongoing basis. The Physical Therapist revises the POC based on evaluation ongoing assessment data. The Physical Therapist works in consultation with the physician and as part of an interdisciplinary team. The Physical Therapist plans and provides appropriate treatment for neuromuscular neurological and orthopedic disorders based on medical referral and therapy assessments. The Physical Therapist assists instructs patient family in transfer techniques appropriate to patient abilities. The Physical Therapist instructs on use of equipment such as walkers wheelchair canes braces prosthetic devises and appliances in a safe manner. The Physical Therapist utilizes appropriate therapy equipment specific to patient diagnosis and needs. The Physical Therapist orders appropriate rehabilitative equipment to meet patients needs and adapts equipment appliances to meet home limitations requirements. The Physical Therapist provides appropriate patient family education regarding therapeutic procedures based on the patient s age condition culture and learning readiness. The Physical Therapist performs and oversees other team members when applicable in the performance of patient care that is consistent with patient s needs and desires. The Physical Therapist applies concepts of infection control and universal precautions in coordinating performing patient care activities to prevent contamination and transmission of disease. The Physical Therapist accepts clinical assignments that are consistent with education and competence to care for patients. Qualifications Education Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program. Licenses Certification Current State License as a Physical Therapist. Proof of current CPR and Hepatitis profile. Current driver s license. Must have reliable transportation and insurance. Experience One - two years experience as a Physical Therapist in an acute care or rehabilitation setting previous home care experience preferred.
