TARGETED CASE MANAGEMENT

Looking for skilled Targeted Case Manager s to provide case management to children adolescents and adults. Experienced TCM s need to be proficient in note writing and have excellent documentation skills. Please have your live scan and ACHA training (ACHA training provided if needed) complete upon applying for position. Requirements Have a bachelor s degree from an accredited university or college with a major in counseling social work psychology criminal justice nursing rehabilitation special education health education or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development) A minimum of 1 year of full time or equivalent experience working with adults experiencing serious mental illness OR Have a bachelor s degree from an accredited university or college and 3 years full time or equivalent experience working with adults experiencing serious mental
