Home refinance loan needed

I am looking for a refinance or cash out loan(HELOC, etc.) on my home in Republic, MO. Want to get $25, 000 cash out. I owe $63, 500 and house will appraise for $110, 000+ . We had some past credit problems and fell a little short in traditional lending. I prefer a lender that is willing to do a second mortgage on the home or a straight HELOC. Will consider total refinance with decent terms. This will be a bill consolidation, vehicle purchase, and home improvement loan. Exit strategy is to consolidate and pay off some debt and then refinance it again in 2-3 years once our scores are improved by the consolidation. Credit score 600-645. This home was built new in 1985 by my family and I have owned it since 2004. I do not reply to lenders that require up-front fees. All fees must be rolled into the loan at closing. This is a good loan and I can lay it out for the right private money people. I have been in the finance business myself in the past.
