JOB DESCRIPTION SunLife Home Health is seeking a compassionate enthusiastic and dedicated Team Lead at one of our associated facilities. Responsibilities Essential Functions Monitor and supervise caregiving team. Follow the instructions of the professional nurse therapist in providing care. Provide assistance with hygiene such as bathing oral care and dressing. Perform and record accurate measurements (i.e. vital signs or intake output as instructed in the care plans). Observe and report any safety hazards found in the client s home or any significant observations regarding the client. Attend staff meetings attend meeting in-service. Report patient complaints to the nurse. Maintain patient confidentiality adheres to HIPPA requirements and agency policy and procedures manual. Possess the ability to follow written and oral instructions. Report abnormal findings in patient s conditions as observed and per care plan to nurse. Comply with HIPAA regulation. Follow Infection Control Policy. Minimum Qualifications Minimum 2 years experience in home health Prior leadership of supervisory experience Maintain professionalism enthusiasm reliability and responsibility when working with clients and other staff Experience with medication management Strong organizational and communication skills Proficiency in English (speaking reading writing) Bi-lingual is a plus Flexibility and adaptability exhibiting qualities of patience and concern for the needs of elderly and or sick clients in a variety of situations Current First Aid and CPR certification Copy of current negative Tuberculosis test Ability to receive Fingerprint Clearance Card Must have reliable vehicle Must be available to work 7am-7pm (some weekends required) Please submit your resume along with a completed application.
