Physical Therapist PT Job Description Under the direction of the Rehabilitation Services Coordinator the Physical Therapist develops and implements a progressive rehabilitation program for clients in facilities in accordance with the principles and practices of physical therapy and the client s needs. Serves as case manager and supervises home health aides and physical therapy assistants when appropriate. Meets the goals and needs of the program and provides quality care by assessing and interpreting evaluations and test results determining physical therapy treatment plans in consultation with physicians or by prescription. Helps patient accomplish treatment plan and accept therapeutic devices by administering manual exercises instructing encouraging and assisting patients in performing physical activities ambulatory functional activities and daily-living activities and in using assistive and supportive devices such as crutches canes and prostheses.
