The Case Manager oversees the provision of services and supports to individuals in accordance with each individual s identified outcomes as well as The Arc s Mission Statement and Guiding Values. This includes developing each person Habilitation Treatment Plan assuring all documentation of service provision and assuring that services meet the goals and objectives as outlined in each planning document. The Case Manager is accountable to the person receiving supports their team and the agency. Education and Experience Bachelors degree in human services preferred with a desired concentration of study related to developmental disabilities mental health and or social work. Two years previous social services work experience with case management responsibilities including one year in the field of developmental disabilities. Any equivalent combination of related education and experience might substitute for a degree requirement in addition to the two years of social service work experience required above. Life experience with individuals with developmental disabilities mental health may be considered in place of social services work experience requirement on a case-by-case basis.
