I ____________________________ HAVE READ THE AGREEMENT FOR ADOPTION DESCRIBED BELOW AND AGREE TO ABIDE BY IT.1. The animal will be given a good home with food water a shelter and humane treatment daily. 2. If the animal is to be chained it will be wearing a collar and on a 20 foot chain or longer. 3. The animal will be returned to the animal shelter or found a good home if the owner is not able to provide a good home for the animal. 4. The animal will be taken to a licensed veterinarian for shots and proper care. 5. The Animal shelter has the right to investigate the new home and to remove the animal if the home is found to be unsuitable. 6. The adoption fee is 35.00 which goes toward paying for the spaying or neutering of the animal. 7. The adoption fee will be refunded if the animal becomes ill and dies within 14 days after adoption and the shelter is notified within that time. 8. The animal will be spayed or neutered as soon as possible and not be allowed to breed. This is Georgia Law 4-14-1. You can be fined 200.00 if not done. 9. The Betty Gambrell Animal Shelter agrees to release any shot records or papers on the animal to the new owner. 10. The shelter agrees to adopt a healthy animal free of any know health problems to the new owner.
