Description The Director of Case Management is responsible for the effectiveness of the case management program at Rockdale Medical Center and is accountable for it s outcomes to senior management. The Director is to build maintain and manage a case management staff that is adept and efficient at assertively managing patient cases through the continuum of care to achieve hospital clinical and financial outcomes as identified in its annual strategic and or performance improvement plans.
