ESKATON REGISTERED NURSE CASE MANAGER ESKATON HOME HEALTHCARE JOB SUMMARY The Registered Nurse manages all aspects of patient care and provides skilled nursing care to patients in their home. This includes direct patient care supervising health aides referring to community resources and or utilizing resource materials as needed. Includes participation in case conferences and staff meetings completing paperwork in a timely manner conferring and meeting with physicians discharge planners and other community persons as needed or directed. Monitors quality of service and discipline providing information on patients condition to physician and supervisor. Resolves patient family concerns.
