Description RN Field Case Manager The RN Field Case Manager will manage the care of members both telephonically and in their homes or other community settings by completing in-person health assessments and coordinating patient care focusing on disease management and keeping members healthy and independent. The RN Field Case Manager will provide clinical and medical management services including case management health assessments interventions and discharge planning.
