Status Full Time (40 Hours)Schedule DaysReporting Relationship Receives direct and continuing supervision from the Manager Director.General Summary of Duties Provides clinical supervision to nursing staff and OPWDD AMAP s input for performance evaluations to ensure the provision of high quality nursing health related services as directed per individualized needs. Actively participates as a member of the Service Planning Interdisciplinary Treatment Team. Provides meaningful training and educational opportunities to enhance knowledge skills to staff and consumers. Maintains open communication with all. Ensure compliance of all applicable regulations and relevant policies and procedures.Essential Responsibilities or Functions of the Position 1. Provides high quality and respectful direct nursing care for each person as needed to include but not limited to assessment of signs symptoms of illness accompanying people to appointments periodic physical checks etc. Ensures appropriate documentation and prompt follow-through with findings requiring further medical health-related attention.2. Coordinates and ensures the provision of high quality medical and health related services for each person. Responsible for ensuring the scheduling review and follow-through with findings of health related appointments and diagnostic laboratory testing as ordered by the prescribing practitioner.3. Ensures provided services and records are in compliance with all relevant regulatory agency policies and procedures.4. Completes initial direct physical assessment as well as all required new admissions responsibilities per checklist within 30 days of admission to the residence. For people living in an IRA CR the RN is responsible for completing a direct nursing assessment every 6 months. People living in an ICF require a quarterly (every three months) nursing assessment to be completed by the RN. Ensures completion and maintenance of current and effective nursing care plans and health related interventions.5. Reviews documentation as completed by other nursing direct care staff and signs full name title and date indicating review. Ensures appropriate and prompt health related follow-up with identified needs noted in the review of the documentation and keeps manager supervisor apprised of status and completion.6. Provides OPWDD Medication Administration Training and supervision to Approved Medication Administration Personnel (AMAP) per the course curriculum regulatory requirements and agency policies and procedures.7. Provides new employee orientation and ongoing training related to health related needs of the people who live at the residences ensures required training records documentation is completed. Provides in-service training as requested needed based on the needs of the people who live and work at the residence.8. Is an actively involved member of the Interdisciplinary Treatment Team Service Planning Team attending all required ITT SPT meetings annual and special review meetings staff and scheduled nurses meetings. Reviews thorough and up-to-date health status and prepares written summary of information to be reviewed if unable to attend such meetings.9. Prepares all physicians orders in compliance with agency policy derived from regulations and ensure that all components of the maintenance of medications (and other health related supplies) current orders accurate MAR legible label up-to-date formulary etc. correctly match.10. Maintains ongoing routine communication with the manager (at weekly meetings) all service providers (including day programs as needed) and member of the ITT SPT other nursing staff physicians and AMAP s.11. Implements and maintains programs of health and safety procedures including infection control.12. Attends all required requested training.13. Promotes the agency s Vision Mission and Philosophy.14. Provides residential on-call nursing responsibilities as assigned during evening overnight weekend and holiday hours to ensure availability to provide guidance and direction to those requiring assistance per the Memorandum of Understanding between OPWDD and the NYS Board of Education.15. Performs all other necessary duties relevant to the responsibilities and function of the position as requested by the manager director.Typical Working Conditions Combination of office residential and community settings. Flexible schedule including some weekend and evening hours. Exposure category 1 (OSHA Bloodborne Pathogens).Typical Physical Demands Requires driving sitting standing walking running bending and lifting supporting up to 50 pounds. Requires sufficient strength and stamina (mentally and physically) to assist with implementation of physical interventions as needed. Requires eye-hand coordination and manual dexterity sufficient to operate a keyboard photocopier telephone and other office equipment. Requires legible handwriting for written documentation and tracking on a regular basis.Knowledge Skills and Abilities Must be efficient organized and have excellent observational skills and ability to handle stressful situations while maintaining professionalism. Patience empathy understanding and acceptance of individual diversity are a must. Must possess the energy and necessary skills to initiate develop and implement a variety of activities on a daily basis. Must maintain a valid Driver s License and driving record that meets Pathways insurability guidelines to operate agency owned vehicles.Education and Experience A Registered Nurse licensed and currently registered by the New York State Department of Education with specialized training or one year experience working with people who are developmentally disabled.Criminal Background Check Must meet the requirements related to Criminal History Record checks that became effective April 1 2005 which requires prospective employees to be fingerprinted if they have regular and substantial unsupervised or unrestricted physical contact with people receiving services in our programs regulated by OPWDD.Apply Now Complete an online application at www.pathwaysforyou.org and include a copy of your resume
