You ll work alongside a dedicated team of clinical staff and supportive leaders while you grow your career.THRIVE AS A QUALIFIED INTELLECTUAL DISABILITIES PROFEESIONAL Hiring Wage 21.75 Hourly Work closely with facility employees to ensure consistency in service design and delivery Participate and lead Individual Supported Care Meetings to assist in creating Implementing revising and reviewing of measurable goal data for Individuals Plan of Care and Behavioral Support Plans Complete and review Individuals Supported Comprehensive Function Assessments Create Action plans designed to implement and measure Facilities Program goals in correlation with state and federal guidelines licensing and certifications Collect and Review data related to Individual Plan of Care and Behavioral Support Plans as well as obtain and enter documentation related to treatment of Medical Behavioral and Nutritional needs Communicate observations of facility programs Individual Plan of Cares and Behavioral Support Plans while collaborating with internal team members on best approaches for successful goal retentions in areas related to conflicts between programmatic medical dietary and vocational aspects Engage and Train employees on best practices company policies and required state or federal trainings Participate in Program on site visits to ensure functional program services are offered in a safe and productive environment Develop and maintain effective professional relationships with all internal and external consumers Qualifications Bachelor s degree in human services related field and one year related field experience in the Human Services Industry Skilled Excellence in written and oral communications Knowledge of State ICF guidelines and regulations preferred Experience with Facilities State Licensing and Recertification Policies and Practices In depth knowledge and experience in creating implementing and measuring Individual s Plan of Care Behavioral Support Plans and Comprehensive Function Assessments Experience in Presentations for Staff training and Support Meetings Other education experience training licensure or certification per state requirements or as required by applicable regulations. Current driver s license car registration and auto insurance. Strong attention to detail organizational skills and the ability to multi-task to meet deadlines. Effective communication skills to create effective relationships with internal and external consumers. A reliable responsible attitude and a compassionate approach.
