Always.Your success is the focus of our entire business.We are hiring for a civilian Pain Management Licensed Practical Nurse (LPN) Licensed Vocational Nurse (LVN) to join our team of talented professionals who provide health care services to our veterans and their families at Naval Hospital Jacksonville FL in the Pain Management Clinic. Benefits Day Schedule Monday through Friday excluding the day of observance of federal holidays for a nine (9) hour shift between the hours of 0600 to 1830 Excellent financial package designed to attract compassionate top performers Comprehensive benefits package to include major medical dental and vision plans 401k Paid time-off and holidays The Licensed Practical Nurse Licensed Vocational Nurse (LPN LVN) Pain Management must have the following qualifications Degree Education Be a graduate from a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) program accredited by the Accreditation Commission for Education in Nursing (ACNE) Licensure Possess a current unrestricted license to practice as an LPN LVN in any one (1) of the fifty (50) States the District of Columbia the Commonwealth of Puerto Rico Guam or the U.S. Virgin Islands Experience Possess one (1) year of clinical work experience as a full-time LPN LVN within the preceding three (3) years Summary of the responsibilities for the Pain Management LPN LVN The Pain Management Licensed Practical Nurse Licensed Vocational Nurse will provide care coordination using specific criteria and guidelines as mandated by the Department of Defense (DOD) the Bureau of Medicine and Surgery (BUMED) and TRICARE directives coordinating with the local Medical Home Port (MHP) clinic utilization and Referral Management policies and obtains additional clinical data as required. Consult coordinate and collaborate with MHP team members referral management team members specialty care clinics outside care professionals agencies organizations and other ancillary support systems to assist in the care coordination of MHP patients. Identify patients in need of care coordination within the MHP teams through screening patients with a pattern of high risk unstable and resource intensive utilization of Military Treatment Facility (MTF) or network services such as chronic pain patients. Responsible for coordination of outpatient care services of patients belonging to the MHP clinic area of responsibility that are discharged from other facilities. Follow up with MHP team members regarding review of patient charts medical records and other documents utilized in the treatment and care of MHP patients in need of care coordination. Participate in an on-site designated Extension for Community Healthcare Outcomes (ECHO) tele-mentoring training program.
