Status Full Time (37.5 Hours)Schedule Monday - Friday 8 30am-5pmNATURE OF WORK Primary Function Responsible for the provision of Home Health Care casework services to qualified individuals. This position is responsible for the provision and or arrangement of an array of Home-Health Care services.KNOWLEDGE SKILLS AND ABILITIES Good knowledge of the practices and procedures of social casework and of social factors affecting individuals and families good knowledge of interviewing and counseling techniques good knowledge of the methods used in the development of case plans working knowledge of the Federal State and Local laws affecting the social casework program working knowledge of social health vocational and other related agencies and programs in the area available for assistance and referral ability to establish and maintain effective working relationships with a wide variety of persons including clients social workers medical and legal professionals government and law enforcement officials and members of the general public ability to assess clients needs and develop sound and effective Home Health Care plans to meet those needs ability to deal with a wide variety of people with an extraordinary amount of tact and courtesy ability to keep case records and prepare reports ability to organized and plan work effectively ready access to transportation to meet field work requirements good powers of observation and analysis initiative and resourcefulness creativity and imagination emotional maturity good judgment.QUALIFICATIONS Bachelor s degree in Psychology Social Work Health Care or a related Human Service field with preference given to direct experience working within the Home Health Care model of Services.
